We currently have two openings at CCS!
One is for a Practice Transformation Coach
The Coach will assist in engagement and outreach activities to primary care practices within the CCS Network; support practices through the “five phases of transformation” outlined in CMS Practice Transformation Grant; lead practices in QI trainings and collaborative activities, ongoing improvement analysis, and development of strategies and best practices to drive improvement. The Coach will ensure practices have access to and are able to implement CCNC population management tools such as: evidence-based best practice guidelines; quality improvement strategies around change management, systems engineering and rapid-cycles tests of change; care coordination and self-management; and how to use data and analytics to support transformation.
Preferred Education/Experience: Clinical knowledge helpful; RN, BSW/MSW preferred. Two plus years of relevant and proven experience in quality improvement, care coordination, and engagement strategies a plus.
The second opening is for a Social Worker
A social worker is needed in Lee County. Must have BSW, MSW preferred, behavioral health care experience a plus. Social worker will coordinate care for Medicaid patients in the Lee County region. Applicant must be computer literate. Please note: Applications without BSW or MSW will NOT be considered for this position.
